چکیده
مقدمه و هدف: درمان­ های غیردارویی روش ­هایی برای کنترل درد کودک در حین انجام اقدامات درمانی مختلف است. بسیاری از مطالعات، روش­ های مختلفی پیشنهاد کرده ­اند. با این وجود هنوز مطالعه ­ای به بررسی این موضوع که در هر رده سنی چه روشی مناسب می ­باشد نپرداخته است. از این­رو مطالعه­ ی حاضر با هدف مرور نظام­ مند مطالعات کارآزمایی بالینی روش­ های غیردارویی کنترل درد در کودکان انجام شد.
روش کار: مطالعه مرور نظام­ مند حاضر براساس راهنمای PRISMA برای مطالعات مروری انجام شد. این مطالعه به مرور مطالعات کارآزمایی بالینی کنترل شده در ارزیابی اثربخشی هرگونه مداخله غیردارویی (در طی انجام مداخله دردناک) در مقایسه با مراقبت ­های معمول برای بهبود تجربه اطفال از درد انجام شد. جستجوی سیستماتیک ادبیات مرور شده توسط دو نفر از محققان این مطالعه، تا سال 2020 انجام شد. این جست و جو در پایگاه­ های PubMed، کتابخانه کوکران و Web of Science، Scopus و Science direct انجام شد. از مقیاس اصلاح شده جداد برای بررسی کیفیت مطالعات استفاده شد. از Microsoft Excel برای استخراج جزئیات مطالعه استفاده شد.
یافته ­ها: نتایج یک بررسی مقدماتی نشان داد که ناهمگنی زیادی در انواع ابزارهای مورد استفاده به­ عنوان روش غیردارویی کنترل درد و ابزار­های مورد استفاده جهت ارزیابی تجربه بیمار وجود داشت؛ بنابراین سنتز یافته ­ها به­ صورت کمی صورت نگرفت. در مطالعه حاضر تعداد 24 مطالعه با مجموع 2653 شرکت ­کننده بررسی شدند. در مورد نوزادان پره­ ترم، 5 مطالعه با مجموع 378 نوزاد بررسی شد. مطالعاتی که به بررسی نوزادان ترم و شیر­خوار پرداخته بودند، سه مطالعه با مجموع 460 نوزاد ترم یا شیرخوار بود. مجموعا 16 مطالعه به بررسی 1815 کودک (نوپا، کودک در اوایل کودکی، کودک در دوران میانه کودکی و نوجوانان) بررسی شدند. تحریکات چشایی و مک­ زدن بیشتر برای نوزادان پره­ مچور مناسب بود. آغوش مادر گزینه مناسبی برای نوزادان ترم بود. در رده های سنی بالاتر روش­ های مختلف انحراف کارایی خوبی نشان دادند؛ اما نتایج در مورد پخش موسیقی متناقص بوده است.
نتیجه گیری: مطالعه حاضر نشان داد استفاده از روش ­های مختلف کنترل درد باید با توجه به سن کودک انتخاب شود و با توجه به وجود گزینه­ های مختلف با کارایی مناسب و عدم وجود خطر در استفاده از روش­ های غیر­دارویی، با توجه به شرایط کودک از یک یا چند روش انحراف فکر مناسب استفاده شود.
